Online CHNA Survey Can Help FHN Improve Healthcare in the Community

July 7, 2025 – Every three years, FHN Memorial Hospital (FHN) conducts a Community Health Needs Assessment (CHNA) to identify community health needs and create a plan to address those needs. FHN is asking people in our communities to go online and complete the short survey, which is completely anonymous.

"The more people who complete the survey, the better our information will be," said FHN President and CEO Mark Gridley. "The information we gather will help us to develop an action plan to address our communities' needs over the next three years."

The 2025 CHNA Assessment and the associated Implementation Strategy document are available online at www.fhn.org/CHNA. The deadline to complete the survey is July 31.

The CHNA is required by Internal Revenue Code, section 501(r) and the Affordable Care Act to gather information from community leaders and stakeholders across a broad range of interests. For the purposes of the CHNA, the FHN service area is defined as Stephenson, JoDaviess, and Carroll counties in northwest Illinois.

Surveys measure participants' perspective on a range of issues that affect the population's health and well-being, including community resources, barriers to healthcare providers, and reasons for high rates of disease and mortality. In addition to the completely anonymous survey, FHN will collect secondary data on a broad array of health indicators and demographic information from databases created by other agencies or organizations.
