Sports Physical Clinic - Orangeville, August 3, 2010

$20 sports physicals meet IHSA requirements for all sports. They do not meet the requirement for school physicals. Proceeds are donated to the school. No appointment needed.

Please download the FHN Consent for Medical Care form and the IHSA sports physical form.

Fill out the Consent for Medical Care form in full if you will not be able to accompany your child to his or her sports physical. (It is not needed if you will be present at the physical.)

Fill out the top section of the IHSA form with your student’s information before you go to the physical.
